Immunization Submission



Submission of immunization records to Student Health is required so that we can verify that you satisfy the Minimum Student Immunization Requirements for the University.

Immunization information can be electronically entered by students using on-line forms available at the Online Student Health portal.   (For help accessing the Online Student Health portal, go to Online Portal Services).

After completing electronic entry, you MUST also send an official copy of your immunizations via US mail or by FAX (412.383.1846).

For students living in University Housing, you are required to submit/upload your COVID Vaccination status (Vaccine records or Exemption form) by July 31.

Failure to provide proof of adequate immunization (MMR, Varicella, Meningitis) by October 1, will prevent your student from registering for the upcoming Spring Term.
