For a Telemedicine Appointment

  16. Eye concerns
  17. Fatigue

For Telemedicine Evaluation to Consider an InPerson Appointment

  1. Gyne Problem (vaginitis, pelvic pain)
  2. IUD, Nexplanon insertion to be scheduled by L.W.
  3. IUD removal
  4. Nexplanon removal to be scheduled by L.W
  5. Annual (≥ 21 y.o.)
  6. Musculoskeletal complaints
  7. Rash
  8. Physical Exam
  9. Eye complaint (eye pain, visual disturbance)
  10. Abdominal Pain
  11. UTI with any atypical symptoms (vaginal symptoms, back pain, F/C)
  12. Chest pain (without URI symptoms)
  13. Ingrown toenails
  14. Abscess I&D
