Disability Shuttle Request Form
The Disability Shuttle is available for individuals with temporary injuries or permanent disabilities if their transportation needs cannot be met with the University’s bus and shuttle system. All campus shuttles are fully accessible. Please check the University routes and schedules before submitting a request for the Disability Shuttle.
Please submit your rides based on your class schedule. Only rides within the SafeRider Boundaries will be considered for approval. The Disability Shuttle Request Form is designed to provide two rides per day (one ride to campus, one ride from campus). These rides will remain the same every week for the duration of your time on the shuttle.
All ride requests require a 24 BUSINESS HOUR PROCESSING PERIOD. You will be required to initial and return a statement regarding the Disability Shuttle policy prior to your first ride. Rides are not valid until this policy statement is initialed and returned.
